Request an Absentee Ballot

The form below is the first of two steps necessary to register to request and absentee ballot. Completing the form below and clicking "Generate My Application" will provide you with a completed absentee ballot request. You will need to print the generated form, sign it and date it, and mail it to the address provided on the printout.

This simplified web application does not include provisions for military, overseas, or hospitalized electors. If you are a military, overseas, or hospitalized voter, please use the full Wisconsin Application for Absentee Ballots. Instructions for that document are included in the download.

To request a Wisconsin absentee ballot, you must be a registered Wisconsin voter.

Your Name

You must provide your full legal name for your registration to be accepted. Leave blank any information that does not apply to you.

Your Address

You must provide your home address in the first address row to confirm your elibility to vote. If you receive your mail at a different address than the address where you live, you must provide your mailing address in the second row. If you receive your mail at the address where you live, leave the second row blank.
